Kindergarten:

1. All students need photo taken for yearbook & ID even if not purchasing them.

(Bring check payable to “Lifetouch” if you would like to order photos)

2. Verification of student information for Infinite Campus at registration

3. Pay fees: (Classroom consumables) (See fees schedule on line).

Returning 1^st-2^nd grade:

1. Information sheets- Vaccine Requirements- IMPORTANT!

2. Forms to print and return completed on registration day:

· Pick-up Authorization

· Lifetouch Photo Flyer with payment if ordering (All students need photo taken for yearbook & ID even if not purchasing)

· Volunteer Form

3. Form to be filled out on registration day:

• Verification of student information for Infinite Campus
• Emergency Health card (only available on May 2^nd)
• (Please bring doctor, dentist, hospital, and emergency contact information with you in May)

4. Form to bring with you, from your doctors office:

• Updated immunization record (1^st gr. only for second Varicella (Chickenpox)

New 3^rd Grade to P2P:
