Returning 6^th Grade:

1. Information sheets- Vaccine Requirements –IMPORTANT!

2. Forms to print from website and return on registration day

• Lifetouch Photo Flyer (All students need photo taken for yearbook & ID even if not purchasing)
• Internet Use Form
• Student Conduct
• Volunteer Form

3. Forms to be completed on registration day

• Verification of student information for Infinite Campus
• Emergency Health card (available on August 5^th) (Please bring doctor, dentist, hospital, and emergency contact information with you in August)

4. Info. to bring with you from your doctors office- Updated immunization record

5. Last stop- Pay Fees (See fees schedule on line).

Returning 7^th & 8^th Grade:
