The Peak to Peak health room is required to maintain updated immunization records for every student at Peak to Peak.
Immunizations are an important part of our children’s health care, and Colorado law requires that children going to school be vaccinated to prevent vaccine-preventable disease. As a parent/guardian, it is important to know that in addition to the vaccines required by the Colorado Department of Public Health & Environment (CDPHE) for school entry, there are vaccines that are recommended by the Advisory Committee on Immunization Practices (ACIP). This is the immunization schedule that will best protect your child from even more vaccine-preventable diseases. The CDPHE also offers a slide presentation on vaccine-preventable diseases and how vaccines work.
Please discuss your child’s vaccination needs with your child’s doctor or local public health agency.
Per CDPHE ALL STUDENTS entering Kindergarten are required to have the following:
- 2 MMR doses
- 3 Hep B doses
- 4 Polio (OPV/IPV) doses
- 5 DTaP doses
- 2 Varicella (Grades K-6) / 1 Varicella (grades 7-12) or laboratory-confirmed immunity.
Per CDPHE ALL STUDENTS entering 6th grade are required to have the following (regardless of age):
- 1 Tdap booster
Under Colorado law, a student may be exempted from immunizations if they have submitted a signed medical, religious or personal exemption (non-medical). A new exemption form must be turned in each school year.
Please email immunization records/exemptions to firstname.lastname@example.org, or bring along with you to registration!FORMS
