Immunizations
The Peak to Peak health room is required to maintain updated immunization records for every student at Peak to Peak.
Immunizations are an important part of our children’s health care, and Colorado law requires that children going to school be vaccinated to prevent vaccine-preventable disease. As a parent/guardian, it is important to know that in addition to the vaccines required by the Colorado Department of Public Health & Environment (CDPHE) for school entry, there are vaccines that are recommended by the Advisory Committee on Immunization Practices (ACIP). This is the immunization schedule that will best protect your child from even more vaccine-preventable diseases. As of July 1st 2021, the process of submitting a nonmedical exemption has changed. There are two ways to submit a nonmedical exemption. See the exemptions section below for more information.
Please discuss your child’s vaccination needs with your child’s doctor or local public health agency.
Per CDPHE, all students entering Kindergarten are required to have the following: CDPHE K-12 SCHOOL-REQUIRED IMMUNIZATIONS
- 2 MMR doses
- 3 Hep B doses
- 4 Polio (OPV/IPV) doses
- 5 DTaP doses
- 2 Varicella (Grades K-6) / 1 Varicella (grades 7-12) or laboratory-confirmed immunity.
Per CDPHE, all students entering 6th grade are required to have the following (regardless of age): CDPHE Tdap Requirement
- 1 Tdap booster
Exemptions
Nonmedical Exemptions
- Submit the Certificate of Nonmedical Exemption with a signature from an immunizing provider who is a medical doctor, doctor of osteopathic medicine, advanced practice nurse or delegated physician’s assistant, or

Submit the Certificate of Nonmedical Exemption received upon the completion of CDPHE’s Online Immunization Education Module. You can complete this form for more than one child without having to retake the course.
- Parents of students in grades K-12 claiming a nonmedical exemption must submit one annually. Nonmedical exemptions expire June 30th each year. If you submit a Certificate of Nonmedical Exemption on or before June 30th, it will not be valid for the upcoming school year unless you submit the exemption during early registration.
Medical Exemptions
- A Certificate of Medical Exemption is used when the physical condition of a student is such that vaccination would endanger their life or health or is medically contraindicated due to other medical conditions.
- Students with a completed Certificate of Medical Exemption (signed by a medical doctor, doctor of osteopathic medicine, advanced practice nurse or delegated physician's assistant) need to submit this Certificate only once unless the student’s information or school changes.
- You can submit a Certificate of Medical Exemption with CDPHE to include in the Colorado Immunization Information System (CIIS). Check with your student’s school, child care or college/university to find out if they want the printed copy of the Certificate of Medical Exemption or, if they can access the “submitted” document from CIIS.
- Per Colorado Revised Statutes 25-4-2403(2.5), immunizing providers must submit medical exemption data to CIIS.
