Health Room Tips at Registration

Please bring the following with you to registration:
 
  • Immunization records indicating your student is up to date with their immunizations OR an exemption signed for personal, medical, or religious reasons. The State of Colorado and BVSD require that the Health Room keep an up to date record for each student enrolled at Peak to Peak each school year.

  • If your student requires medication at school, please see the Medications page for instructions on which forms you will need to bring to registration.

  • Medications may be brought to registration or the Health Room by an adult - DO NOT SEND MEDICATIONS TO SCHOOL WITH YOUR STUDENT! All medications must comply with the BVSD Medications In School Guidelines.

  • Any information regarding a chronic health condition so we can work on a plan of care together. Please contact the Peak to Peak School Nurse Consultant. Together with you, she will determine if a Health Care Plan is necessary.
